The Improving the Health of Floridians (IHOF) program focuses on patient interventions for providers focused on pre-diabetes, diabetes, hypertension, and cholesterol. A key focus is providing patient resources beyond the physician’s practice that help educate and motivate patients to improve their health status. One of those resources is Lifestyle Change Programs that are designed to provide patients with effective tools and techniques to manage their chronic conditions using the latest evidence-based information.

National Diabetes Prevention Program
The National Diabetes Prevention Program (National DPP) is a partnership of public and private organizations working to prevent or delay type 2 diabetes. Partners make it easier for people at risk for type 2 diabetes to participate in evidence-based lifestyle change programs to reduce their risk of type 2 diabetes.
The Heart Health Plus Program was created with the purpose of reducing health inequity in areas with a disproportionate burden of heart disease and related risk factors. Sixteen county health departments (CHD) have implemented Heart Health Plus activities, including blood pressure self-monitoring (BPSM) programs, within their communities. In addition, CHD staff work with community partners to provide screening and outreach within priority populations. They collaborate with health systems to establish policies and systems that encourage the use of BPSM. They also work to increase referrals to additional self-management and healthy behavior support services, such as Diabetes Prevention Programs (DPP), Diabetes Self-Management Education and Support (DSMES), and Quit Your Way services of Tobacco Free Florida.

The YMCA’s Diabetes Prevention Program is an affordable, high-quality lifestyle change program directly translated from the National Institutes of Health (NIH) Diabetes Prevention Program.
